Health Office Policies
MANDATED PHYSICALS
According to NY State Education Law, students entering Kindergarten, grades 1, 3, 5, 7, 9, 11, sports, working permits, triennially for the Committee on Special Education and all new students to the school district must have a current health appraisal. A health appraisal is valid for a 12 month period. If a health appraisal is not received by a child's private physician by October 1, an exam will be given by the school physician. Dr. Gerald Lauria. This brief exam should not take the place of a yearly exam by your family physician.

ANNUAL SCREENINGS
The State of New York mandates that health related screenings be completed in our schools each year. The requirements are as follows:
For new district entrants (regardless of grade):
Vision: Distance Acuity, Near Vision, Color Perception
Hearing-Pure Tone
Health Appraisal
Vision Screening: Distance Acuity in Kindergarten, grades 1, 3, 5, 7 and 11
Vision Screening: Near Vision in Kindergarten, grades 1, 3, 5, 7 and 11
Pure Tone Hearing Screening: Completed in Kindergarten, grades 1, 3, 5, 7 and 11
Scoliosis: Completed with Boys in grade 9 and Girls in grade 5 and 7
Health Appraisals: Physicals are also required in Kindergarten, grades 1, 3, 5, 7, 9 and 11 and students participating in sports.
Screenings may be requested at any time, in any grade level, by parents and/or your child's teacher because of concerns based on observations in home or classroom settings.
If there are any unusual findings, or cause for concern, the school nurse in your child's building will contact you. In the event that additional evaluation is needed, a referral form will be sent home for you to take to your private health care provider.
MEDICATION ADMINISTRATION
The East Aurora Board of Education has a policy regarding medications in school. It was adopted because of an increasing awareness and concern for the safety of children taking prescription as well as over-the-counter drugs.
When circumstances make it necessary for the child to take medication during school hours, the following procedure must be followed:
1, The parent must make a written request for administration of the medication.
2. The prescribing doctor must also make a written request stating the name of the medication, dosage and time to be taken.
3. The medication must be brought to the Health Office BY THE PARENT in the original container from the pharmacy which dispensed the prescription.
This policy applies to even the occasional request for aspirin, acetaminophen or ibuprofen.
All medications will be kept in the Health Office and dispensed from there. (Physician specified authorization can be obtained for a student to self carry medication.( inhalers, epipens, etc.)
Written requests must be renewed each school year.
