Physical Exams 
All students must pass a physical examination and have a signed Extracurricular Rules & Permission Form on file in the athletic office in order to participate in sports. The form must have a written parental signature. The school nurse will arrange a date for a physical examination when the signed parent permission slip is returned to the health office. NO EXAMINATION WILL BE GIVEN WITHOUT THIS SIGNED SLIP.
Students having their personal doctor perform the physical examination should submit a Health Appraisal Form completed and signed by their family physician. These forms are available in the health office or high school main office.These forms must be approved by the school physician/school nurse after the personal physician’s approval and prior to participation.
Physicals for fall sports will be held at the middle school health office from 8:15 am - noon on the following dates:
- Friday, May 17th
- Monday, June 24th
Students need to see the nurse in their building to schedule an appointment for the dates above.
Summer physicals will be held at the high school health office from 8:30 - 10:30 am on the following dates (no appointment is necessary):
- Monday, August 5th
- Friday, August 9th
